HOURS: 40 hours work week; 8 hour shifts with some weekend shifts. Good Shepherd is seeking a passionate and dedicated individual to join our team as an experienced Charge Nurse - Hospice and Palliative Care How You Will Make an Impact Reporting to the Clinical Nurse Manager, the Charge Nurse is responsible for providing day to day clinical support to the Hospice care team and assisting with the day-to-day management and administration of the Hospice. You will promote and model positive relationships within the interdisciplinary team and our community while supervising and coordinating Nursing care delivery, and you will play a key role in the day-to-day operations of the nursing team while providing culturally safe and quality patient centered care. Duties include: Coordinate and supervise patient care as the nurse in charge, ensuring adherence to hospice policies, nursing best practices and CNO Standards of Practice. Actively participate in patient care as needed, assessing patient needs, planning and delivering treatment, and evaluating outcomes. Manage staffing calls, schedules and monitoring, workload to ensure optimal patient care and staff performance. Serve as a mentor and resource for all nursing and PSW staff, promoting professional development, mutual respect, collaboration, accountability and teamwork. Communicate effectively with patients, families, and healthcare team members, ensuring information is accurately conveyed and concerns are addressed promptly. Participate in quality improvement initiatives, identifying opportunities for improvement and implementing evidence-based practices to enhance patient outcomes. Ensure compliance with all regulatory and accreditation standards. Participation in On Call support for after hours clinical and staffing support to the Hospice on a rotational basis Help facilitate and co-ordinate admissions, discharges and resident flow within the Hospice in collaboration with the care team Administrative reporting on performance indicators as per accreditation and Ministry standards Supports the Orientation process for new staff including in-service education as required, on the job mentoring and modelling skills related to Nursing and PSW practices and standards Supporting medication management and chart audits What You'll Bring to Our Team Graduate of an accredited school of nursing and registered in good standing with the College of Nurses of Ontario as a Registered Nurse is required Current CPR & Basic First Aid certificate required Experience working with individuals and their families in the end of life or with a life limiting illness Strong clinical assessment skills and the ability to work with evidence-based practices related to pain and symptom management and end of life care are required Previous palliative care experience either in hospital based palliative care, hospice or other healthcare settings with knowledge of the death/dying process Proven ability to work as a member of an intradisciplinary team Enhanced Fundamentals of Palliative Care is an asset (completed or actively pursuing) an asset Resourceful problem-solver with strong communication and interpersonal skills, excellent time management and ability to perform assigned duties with attention to detail Demonstrated commitment to professional continuing education Ability to work 8-hour shifts including rotational weekends as scheduled Excellent attendance record required Demonstrated commitment to supporting respectful and rewarding work environment and supporting opportunities to grow and learn Committed to anti-racist/anti-oppressive practice and to the mission and values of Good Shepherd Fluency in a language in addition to English is an asset What We Offer Be a part of a caring team that support our local community and delivers high quality client centered care Support for continuing education and professional development Staff recognition programs along with access to self-care supports and Employee/ Family assistance programs (EAP_ Positive and encouraging atmosphere Make a difference and provide hope Extended benefits Good Shepherd, a ministry of the Hospitaller Order of St. John of God, is an equal opportunity employer operating within an anti-racist and anti-oppression framework.
