Permanent Part-time Social Worker II – Rehab Therapies Department REQUIREMENTS:
· Masters Degree in Social Work
· Registration with the Ontario College of Social Workers and Social Service Workers
· Previous social work background working with clients with disability
· Background in conducting psychosocial assessment and be versed in relevant consent and capacity legislation, the Social Work Code of Ethics and the Heath Care Consent Act
· Demonstrated strengths in interpersonal relations, issue solving and leadership REQUIRED RESPONSIBILITIES: The Social Worker is responsible for completing psychosocial assessments while providing goal orientated support and counseling for patients and families dealing with disabilities such as stroke, limb amputation, spinal cord injury, and life altering issues. The successful applicant must provide consultation with professionals within our organization as well as community health care providers.
